K-LAB DJ Service Booking Form

By submitting this form, I understand and agree to the following K-LAB DJ Service Terms and Conditions:

1. Booking Confirmation. Submitting this form is a request for service only and does not guarantee availability or confirm a booking. K-LAB DJ Service must review and approve the event details before the booking is confirmed.
2. Payment and Deposit. A booking is not confirmed until the required deposit, payment, or written agreement has been completed. The deposit is non-refundable. Any remaining balance must be paid by the deadline provided by K-LAB DJ Service.
3. Event Details. The customer/event host is responsible for providing accurate event details, including event date, time, venue address, guest count, setup location, venue contact information, and event schedule. Any changes may affect pricing, availability, or service approval.
4. Venue Requirements. The customer/event host is responsible for confirming all venue requirements before the event, including load-in rules, parking, power access, setup restrictions, sound limits, vendor rules, permits, and insurance requirements.
5. COI / Additional Insured Requirements. If the venue requires a Certificate of Insurance, Additional Insured certificate, or any insurance-related document, the customer/event host must notify K-LAB DJ Service before the event. Any related cost, fee, or processing charge is the responsibility of the customer/event host.
6. Load-In, Setup, and Parking. The customer/event host must provide reasonable access for load-in, setup, and breakdown. Parking or loading access must be available for K-LAB DJ Service. If parking, loading, elevator access, stairs, long-distance carrying, security clearance, or venue restrictions require additional time or labor, additional fees may apply.
7. Power Requirements. The customer/event host is responsible for ensuring safe and reliable power access near the DJ setup. K-LAB DJ Service is not responsible for service issues caused by venue power problems, unsafe electrical access, or lack of required power.
8. Outdoor Events and Weather. For outdoor events, the customer/event host must provide safe covering, shade, and weather protection for all DJ and sound equipment. K-LAB DJ Service reserves the right to pause, delay, or stop setup/performance if weather, heat, wind, rain, dust, moisture, or other unsafe conditions may damage equipment or create a safety risk.
9. Equipment Safety and Damage. The customer/event host is responsible for maintaining a safe event environment, and may be responsible for damage to K-LAB DJ Service equipment caused by guests, venue staff, negligence, spills, unsafe conditions, or unauthorized handling of equipment.
10. Music Requests and Event Vibe. K-LAB DJ Service will make reasonable efforts to follow the customer's music preferences, must-play list, do-not-play list, and event vibe. Guest song requests are played at the DJ's discretion. Exact song availability, timing, and requests are not guaranteed unless agreed upon in advance.
11. Overtime. Any requested performance time beyond the agreed booking time is subject to DJ availability and additional overtime fees, and must be approved by K-LAB DJ Service and the customer/event host.
12. Cancellations and Rescheduling. The deposit is non-refundable. Cancellation and rescheduling terms may vary depending on the event, package, and agreement. Any additional payments may be non-refundable unless otherwise agreed in writing by K-LAB DJ Service.
13. Safety and Right to Refuse Service. K-LAB DJ Service reserves the right to refuse, pause, or stop service if the event becomes unsafe, unlawful, hostile, or damaging to staff, guests, equipment, or the venue.
14. Liability Limitation. K-LAB DJ Service is not responsible for delays, interruptions, or service limitations caused by venue restrictions, unsafe conditions, power failure, weather, acts of God, emergency situations, incorrect customer information, or circumstances outside of K-LAB DJ Service's control.
15. Final Approval. All bookings are subject to final approval by K-LAB DJ Service. K-LAB DJ Service may decline a booking request if the event details, venue requirements, schedule, safety conditions, or service expectations are not suitable.
